Primary Care Networks comprise of clusters of General Practices who work together to improve health and other care for patients. Together GPs work with social care, pharmacists, mental health and other local health and care providers to deliver local services to the local population.
Newly formed Nottinghamshire PCNs were keen to have information on the health, wellbeing and care needs of their respective populations. Therefore, Nottinghamshire County Council’s Public Health department has led the production of PCN Health and Care Profiles.
A total of 28 PCN Health and Care profiles have been produced for the NHS Nottingham and Nottinghamshire area.
These PCN profiles were last updated in January 2020 and there are currently no plans to update them.
